Important – Please Read Carefully

By submitting this form, you are agreeing to participate in an ocean-based activity with Eco Adventures Limited, and acknowledging that this activity carries inherent risks that may result in injury, property damage, or even death.

Safety Briefing

I have understood the safety briefing and am satisfied in all respects that I would like to proceed with this charter and do so at my own risk.

Risks Acknowledged

I understand that participating in a boat charter and associated activities — such as fishing, swimming, snorkeling, paddleboarding, kayaking or diving — exposes me to various risks, including but not limited to:

• Slips, trips, and falls on or around the vessel including marinas and boat ramps
• Sea conditions such as waves, current, or adverse weather
• Injuries from fishing or diving equipment
• Injuries from landed fish such as fish spikes and bites
• Interactions with marine life such as sharks, rays, marlin, tuna or any other marine species
• Drowning or other water-related injuries

I understand that if I enter the water, I do so entirely at my own risk. Eco Adventures Limited does not supervise or operate any in-water activities. It is not a diving or snorkelling instruction service and does not provide rescue personnel.

If I engage in water-based activities, I confirm I have the necessary skills, experience, and certifications, and I am medically fit to participate.

Release of Liability

I hereby waive, release, and discharge Eco Adventures Limited, its owners, employees, contractors, agents, and affiliates from any and all claims or liabilities for injury, loss, damage, or death arising from my participation in this charter, including those arising from negligence, to the maximum extent permitted by the laws of New Zealand.

This release extends to:

• Any claims by my estate, heirs, next of kin, or assigns
• Any claims resulting from product or equipment failure
• Any rescue or medical expenses I may incur

Damages to Vessel or Equipment

I agree to cover the cost of any damage to the vessel or equipment caused by my negligence, including loss, breakage, misuse, or water damage.

Photography and Video

I understand that photos, videos, and audio recordings taken during the charter may be used by Eco Adventures for marketing and promotional purposes. If I do not wish to appear in this content, I have advised Eco Adventures and understand that reasonable efforts will be made to respect my request.
